License Requirements

Processed foods must provide a photocopy of the bakery, food processor, or confectionary license.
Apple cider and maple syrup vendors must provide a license if using another processor.
Bakery items and other processed foods must provide a copy of license for the certified kitchen
I am applying for:
A $25 non-refundable fee is required for seasonal and daily vendors. The application and vendor fees are used to pay for marketing and operational expenses.

Seasonal Vendor

Submit form and send check made for the vendor fee plus the $25 application fee.
NOTE: Submission of form is equivalent to a signature.

Daily Vendor

Submit form and send check for the $25 application fee. Your daily vendor payment is due before you set up for the first market you attend.
NOTE: Submission of form is equivalent to a signature.
Make checks payable to the South Madison Farmers' Market.
Please attach a copy of your automobile liability insurance to the mailed check. Or e-mail insurance verification to: robert@southmadisonfarmersmarket.com.
